Lecture 14: When Dampness Accumulates – Symptoms and the Distinction Between Internal and External Dampness
In the previous lecture, we explored Cold pathogenic influence (寒邪) and its treatment principles.
From this session onward, we turn to another Yin-type pathogenic factor among the Six Climatic Influences: Dampness (濕邪).
Unlike cold, which constricts and freezes, dampness behaves like a sticky marsh—it slows circulation, weighs the body down, and causes illness to linger.
1. What Is Damp Pathogenic Influence (濕邪)?
Dampness refers to excessive moisture from the natural environment or internal metabolic dysfunction that disrupts normal physiological processes.
Its defining characteristics are:
-
Heaviness (重)
-
Turbidity (濁)
-
Stickiness and lingering nature (黏滯)
Because of these qualities, damp-related disorders often persist for long periods and are slow to resolve. Dampness obstructs the smooth movement of qi and blood, creating a sense of stagnation throughout the body.
2. Signs That Dampness May Be Accumulating
When dampness accumulates internally, the body may exhibit the following signs:
▪ Sensation of Heaviness
The entire body feels heavy, as if wrapped in wet cotton.
▪ Edema
Swelling of the face, hands, or feet may occur easily.
▪ Dull or Yellowish Complexion
The face may appear cloudy or yellowish rather than bright and clear.
▪ Joint and Muscle Discomfort
Pain and stiffness may worsen on rainy or humid days.
▪ General Discomfort
-
Persistent fatigue and sluggishness
-
Difficulty urinating smoothly
-
A lingering sense of internal dampness
These manifestations reflect the obstructive and stagnating nature of damp pathogenic influence.
3. Two Forms of Dampness: Internal vs. External
Traditional medicine distinguishes dampness based on its origin:
| Category | Internal Dampness | External Dampness |
|---|---|---|
| Cause | Weak digestive function leading to fluid accumulation | Exposure to humid weather or damp environments |
| Main symptoms | Abdominal fullness, bloating, diarrhea | Heaviness and weakness in the lower limbs |
| Characteristic sign | Digestive dysfunction predominates | Dark yellow or reddish urine |
▪ Internal Dampness
Internal dampness arises primarily from dysfunction of the digestive system.
Improper diet or weakened metabolic transformation leads to accumulation of fluids within the body.
▪ External Dampness
External dampness results from environmental exposure, such as prolonged time in humid conditions or rain.
Heaviness in the lower body is often prominent.
4. Why Is This Distinction Important?
Dampness rarely acts alone.
It frequently combines with other pathogenic factors, creating more complex disorders.
-
Damp-Heat (濕熱)
Dampness combined with heat may lead to inflammatory conditions or systemic stagnation. -
Cold-Damp (寒濕)
Dampness combined with cold may produce severe joint pain and deeper stagnation.
Therefore, determining whether dampness originates internally or externally is the essential first step in treatment.
Summary
If you experience:
-
A persistent sensation of heaviness
-
A yellowish or dull complexion
-
Worsening discomfort during rainy weather
Dampness may be involved.
-
Diarrhea and digestive issues suggest internal dampness.
-
Heaviness in the legs with darker urine suggests external dampness.
Recognizing the nature of dampness is the foundation for effective treatment.
Preview of Part 17
If dampness is weighing the body down, how can it be removed?
In the next lecture,
“Lecture 15: Specialists in Eliminating Dampness – Qiang Huo, Cang Zhu, Fu Ling and Other Key Herbs”,
we will examine the herbal medicines traditionally used to dry dampness and restore balance.
